This acute myeloid leukemia has blasts with distinct morphology; the bulk of the blasts show a "tail. Because the cytoplasmic spur can be seen in many different directions, it is unlikely that this is a smear artifact. What does cause this abnormality is not known. This AML involves one with a poor prognosis because of the presence of the RUNX1 and TP53 mutation.
