The cells described below are of a 4-year-old child with pre-B acute lymphoblastic leukaemia. A strikingly monotonous presentation of blasts with hardly any cytoplasm visible.

The blasts of this patient (CD34+, cyTdT+) originate from B-cells, as evident by the expression of CD19 and cytoplasmatic CD79a. Their immaturity is further evident by the absence of immunoglobulins, absence of CD20 and expression of CD10. In addition, these blasts display aberrant expression of the myeloid marker CD33.
