A chronic myelomonocytic leukemia with abnormal monocytes that morphologically resemble degranulated promyelocytes. The neutrophils also exhibited degranulation. Immunophenotyping showed mainly monocyte maturation. The total leukocyte count in peripheral blood was

, of which 10% were blasts. In the bone marrow, 17% blasts were counted, making this CMML type 2. Shortly after diagnosis, transformation into an AML occured.
