An acute monocytic leukemia with ~50% blasts and a leukocyte count of

30⋅10⁹/L

. Remarkable are the large, oval/round shaped nuclei enveloped by dark blue cytoplasm with projections and even darker colored edges. Multiple blasts display vacuolisation of both the cytoplasm and the nucleus.

The blasts of this patient only express CD34, CD117, HLA-DR and CD11c. Other monocytic markers are lacking. However, a clear progression to a more pronounced monocytic immunophenotype is visible and indicated by the black arrows. The expression of CD56 is abnormal.
