Except for a single blast, a monoclonal population is not immediately apparent in this preparation (all lymphocytes are shown). What does stand out tremendously is the extreme number of ruptured cells (6x more than what is currently shown). Due to improper production, malignant cells are more fragile than usual and can easily break down when swabbing the blood for a specimen. Immunophenotyping indeed finds a 30% monoclonal B-cell population (90% Lamba+).
