Lymphocytes with a double nucleus are strongly associated with polyclonal persistent B-cell lymphocytosis (PPBL). This benign condition is probably caused by heavy and prolonged smoking; quitting makes the abnormal lymphocytes disappear in many cases. However, the exact mechanism is unknown.
