Atypical monocytes
Monocytes may have Döhle-like structures and/or an abnormally shaped nucleus. The nucleus may be atypically lobulated or display pointed protrusions, making the monocyte appear more like a neutrophil overall. When the majority of monocytes show this atypical morphology, it may indicate the presence of a hematologic disorder (e.g., MDS, CMML or AML).
