A pale patient
A 51-year-old man, with existing neurological problems (including epilepsy), presented to the Accident & Emergency department (A&E) in the early morning. He had been feeling unwell since the night before and had gone home from work early. His responsiveness varied from adequate to slow and he was unable to answer questions properly.
Now he has a fever (40°C), rapid breathing and a poor saturation of 85% on room air. This man is exhibiting classic symptoms of sepsis and he was immediately given a single dose of antibiotics. Initial lab tests were requested. These revealed pancytopenia (reduced numbers of RBCs, platelets and WBCs) and a severely disrupted coagulation.

Microscopic differentiation showed hypogranular cells (figure 1). A bacterial infection is usually accompanied by toxic granulation (hypergranulation), so this finding was unexpected.

Hypogranular promyelocytes were also found. A number of these cells contain groups of purple inclusions. These are so-called "auer rods", or – when present in groups – "faggots". The cells are then called 'Faggot cells'. These rods develop during incorrect maturation. In practice, this is typically a sign of an acute promyelocytic leukemia.

Course
The ER is informed about the faggot cells and the patient is immediately treated with ATRA (Vitamin A). Vitamin A promotes the maturation of promyelocytes to neutrophilic granulocytes. The blood culture later reveals that he also has streptococcal sepsis so treatment with antibiotics was continued. The APL was confirmed by means of immunophenotyping and molecular diagnostics; the population is CD13+,CD33+,CD117+,CD34- and HLA-DR- and positive for the PML-RAR alpha gene mutation (t(15;17) translocation).
This APL is, despite the reduced granulation, not the classical 'hypogranular' variant, which is often CD2+ and CD34+ and is associated with high leukocyte counts
After several weeks of intensive treatment, the man made a complete recovery and was discharged to home.
This case demonstrates that sepsis or acute (promyelocytic) leukemia are not necessarily associated with high leukocyte counts. One also needs to be aware of the existence of variants with reduced granulation, which can hamper morphological identification.
